New York Bariatric Psychological Evaluations is not in-network with insurance plans, and does not accept insurance payments; so you are responsible for the full payment at the time of service.

The fee for this service, including the preparation of the Psychological Report and the sending of the completed report to your Bariatric surgeon, is $199.
